South Central Preschool

This is the preschool application for the 2021-2022 school year. Please be sure to fill the preschool application out completely including the doctor and dentist’s names and phone numbers on the Medical Emergency Form. The form can be filled in and emailed to tlauber@south-central.org, but be aware you will still need to print the medical/physical form and the dental health record for the medical provider to fill out.
